What does a "Secure Message" notification email mean?

If you receive an email notifying you that you have received a secure message this means that the content of our message contains sensitive health information. This message could not be sent securely by email. Login to your Med+Proctor account to safely read this message.

What if I am unable to collect of my records in time or if I am waiting for results?

Please contact your college or university if you require additional time. Only they can grant temporary waivers.

What is an NPI Number?

NPI stands for “National Provider Identifier” and is a unique 10-digit identification number issued to health providers in America by CMS (Centers for Medicare and Medicaid Services (CMS). It is a simplification standard for the Health Insurance Portability and Accountability Act (HIPAA). Covered providers must share their NPI with other providers or any entity that may need billing purposes.

Your doctor should be able to provide this information.

What is Insurance – Self-Pay Billing?

Self-Pay Billing section is asking for an contact information of a financially responsible party in case you have to go see your campus physician. This information is solely for the university to use. Med+Proctor does not use this contact info for any other purpose.

"Men B Not Accepted" what does this tag mean?

Your school does not accept the serogroup B meningococcal (MenB) vaccine. The Meningococcal conjugate vaccine (MCV4, MenA), also known as Menactra, MenQuadfi, and Menveo, is required from within the last 5 years.

"Personal Information Missing" what does this tag mean?

If your account is tagged with “Personal Information Missing” it means that one or more of your uploads is missing your name or date of birth which helps us validate the documents are yours.
Please upload your documents with your first and last name included on them so that our staff can properly review and verify your medical information!

"Referred to Institution" What Does This Tag Mean?

If your account has this tag on it, it means your records are under review by staff from your university. Please monitor your email for additional updates. No action is required at this time.

"Online Exemption request" What Does This Tag Mean?

Your account has been marked as being an online student account. Your school has been notified and you do not need to do or submit anything.

"FLShots data added" What Does This Tag Mean?

If your account has the flag “FLShots data added,” you have purchased the service to pull your vaccine records from the state registry database and upload any vaccination information they have to the Med+Proctor platform.
If you still have any missing vaccination requirements, you will be notified. If you are compliant with your school’s vaccination requirements, then you will be or should be marked as “Verified” and are all good to go. No further action is needed.

"Complete Insurance Billing Page" What does this tag mean?

If you’ve received this flag on your account, you have not completed the required Insurance Questionnaire in your Med+Proctor account. 

To do so, kindly proceed to the insurance section of your Med+Proctor account, located on the right-hand side of the page, and ensure completion of all webpages. Once this is done, your account will be marked as compliant and you will have satisfied the medical requirements for your university.

"Missing UCF health history form" What Does This Tag Mean?

Your account is missing the UCF health history form. The UCF immunization health history form is mandatory for all incoming students. "Conscience Exemption" What Does This Tag Mean?

Your conscience exemption has been received and your school has been made aware. For now, you are all good to go. Please keep an eye out for a message from your school or from Med+Proctor for any futher instructions.

"Meningitis A/C/W/Y Waived" What Does This Tag Mean?

We have received your Meningitis exemption and your school has been made aware. Nothing else is needed from you regarding the Meningitis vaccine at this time.

"Missing UH TB Screening" What Does This Tag Mean?

Your account is missing the UH TB Screening form. You can upload this form in place of a TB test. The TB screening can be any form, as long as it is completed within 12 months of the start of the semester.

"TB Confirmation By US Practitioner" What Does This Tag Mean?

You are missing a signature or confirmation on your TB documentation from a US licensed practitioner. The University of Hawaii System requires that TB tests and screenings be done in the United States or done by a practitioner licensed in the United States. We cannot accept your documents for TB as they currently are.

"Medical Exemption Request" What Does This Tag Mean?

Your medical exemption request has been received and submitted to the school for review. You will be notified once a decision has been made regarding its approval or denial.

Your religious exemption request has been received and submitted to the school for review. You will be notified once a decision has been made regarding its approval or denial.

"Need TDAP vaccine from age 10 or older" What Does This Tag Mean?

Your account is missing a TDAP vaccine from the age of 10 or after. This is a school and state requirement and proof of the vaccine date must be uploaded to your Med+Proctor account.

"Missing TDAP from age 10 or after" What Does This Tag Mean?

Your account is missing a TDAP vaccine from the age of 10 or after. This is a school and state requirement and proof of the vaccine date must be uploaded to your Med+Proctor account.

"TB Outside of the US Not Accepted" What Does This Tag Mean?

Your school does not accept TB tests or TB screenings done outside of the US. The healthcare practice or practitioner must be licensed in the US in order to accept your TB documentation. 
Please have a new TB test or TB screening done with a US licensed healthcare practitioner.

"Missing Date Polio #1" What Does This Tag Mean?

Your account is missing proof of 1 of 4 required Polio dates. 4 doses of Polio are required by your school in order to be considered compliant with immunization requirements.

"Missing QuantiFERON Gold in the US" What Does This Tag Mean?

Your school requires that you upload a QuantiFERON Gold test (TB Blood test) and it must be done in the United States.

"Admin Requests Review" What Does This Tag Mean?

Your account has been escalated for further review. Once the account has been processed, you will be notified of any further actions needed.

"Missing meningitis from w/in last 5 years," What does this tag mean?

The tag on your account, “missing Men A from w/in last 5 years,” means that your university requires a Meningococcal A vaccine administered within 5 years of the start of the semester or classes.
It appears you received your most recent Men A vaccination more than 5 years ago. If you have documentation of an updated Men A vaccination (administered within 5 years), please upload it. If you do not, you will need to receive a Meningococcal A booster.

"TDAP Expired" What does this tag mean?

The tag on your account stating “TDaP Expired,” means that your university requires a TDaP vaccine administered within 10 years. It appears that your most recent TDaP vaccination was administered more than 10 years ago. 
If you have documentation of an updated TDaP vaccination (administered within 10 years) please upload it. If you have not received this vaccine within the required timeframe of 10 years, you you will need to receive a TDaP booster.

First and last name missing

Your institution requires you include your first and last name on all your forms. Please re-upload your forms with your first and last name on them in order for us to be able to approve your documents, given all other vaccination requirements have been met.

Why did I receive notice that I am missing Health Professional Information?

Health Professional Information includes 2 of the 3: Physician/Health Provider Signature, NPI number, and/or office stamp. 
If your University requires both immunization dates and a tb test the Health Professional Information needs to be under BOTH sections. One is not sufficient.

"Backup Documentation With Student Health" What Does This Tag Mean?

If you are seeing this tag on the status page of your Med+Proctor account; this means you have previously submitted immunization records to your school, but you may still be required to submit additional documents to meet the rest of the immunization requirements. Your account will be processed within 24-72 hours of your document submission, and you will be notified if you are required to submit anymore records.

"Conscience Exemption Request" tagged on my account, what does this mean?

If “Conscious Religious Exemption” is tagged on your account this means: your exemption has been successfully received! This tag is listed on your account to alert your school to review your exemption form. This is not an issue and requires no additional documentation at this time.

"English Translation Needed" What Does This Tag Mean?

We will attempt to translate all uploaded documents. The tag, “English Tranlation Needed,” will be tagged on your account if we are unable to translate the documentation provided. Please upload a translated version of your immunization form.

"Health Care Signature" What Does This Tag Mean?

The tag, “Health Care Signature,” means that a form you’ve provided requires a Healthcare signature.

Any form uploaded to MedProctor that has any handwritten information must include a healthcare professional’s signature, office stamp as medical verification of your listed immunizations.

We accept any kind of record/forms like previous high school or four-year college/university, military records or World Health Organization (WHO), or family physician/childhood records.

"Health Professional Information" What Does This Tag Mean?

If you are seeing this tag on your account, then the healthcare provider’s information was not provided on your form. Please make sure all required information is filled out, then re upload the completed form to your Med+Proctor account.

"History of Chicken Pox" What Does This Tag Mean?

If you see this tag, then a support staff member has identified your previous chicken pox/ varicella infection. This tag is placed on your account as a method for your institution to quickly identify registrants that have previously been infected with the chicken pox/ varicella and requires no action in your Med+Proctor account.

"Missing Date HepA" What Does This Tag Mean?

You are missing one or more doses of the Hep A vaccine. You will need to submit proof of having recieved the Hep A series.

"Missing Date Men B" What Does This Tag Mean?

The tag, “Missing Date Men B,” means you are missing a Meningococcal B vaccination. If you have documentation of this vaccination, please upload it. If you do not, you will need to receive a Meningococcal B vaccination.

"Missing Date Polio" What Does This Tag Mean?

The tag, “Missing Date Polio,” means you are currently missing evidence of your Polio vaccination record in your MedProctor account. Your school/ university requires each student to provide proof of a full polio vaccination (4 total doses to meet compliance). If you have documentation of your polio vaccinations please upload them.

"Missing Date Tdap" What Does This Tag Mean?

Tdap is a combination vaccine that protects against three potentially life-threatening bacterial diseases: tetanus, diphtheria, and pertussis (whooping cough). Td is a booster vaccine for tetanus and diphtheria. It does not protect against pertussis. 
Many states laws require colleges students to show proof of having received TDAP within the last 10 years or in some states multiple doses. It appears there is no history of this vaccination present in your account. If you have documentation of receiving a TDaP vaccination please upload it to your MedProctor account. If you do not you will need to receive a TDaP booster.

"Missing Drug Screening" What Does This Tag Mean?

Your account is missing the Drug Screening Form.

"Missing HepB Antibody Doc" What Does This Tag Mean?

Your account is missing the Hep B Titer Document. Please obtain proof of having recieved a Hep B titer and upload this to your Med+Proctor account.

"Missing Insurance Card" What Does This Tag Mean?

Your account is missing the front and back of your insurance card. Please take a photo on your phone or scan and upload this to your Med+Proctor account.

"Missing Med+Proctor Immun Cert" What Does This Tag Mean?

The tag on your account stating, “Missing Med+Proctor Immun Cert,” means that your school/ university requires you to use their school specific form to submit proof of your immunization records. You must have this form filled out, signed, dated and stamped by a healthcare provider.

You can obtain this form in your Med+Proctor account. The steps below on how to download your Med+Proctor Immunization Certificate are copied below:

Once you create your Med+Proctor account, you will need to complete all the prompts and then you will have access to the ‘Documents’ section to download your form.
The option to download your Med+Proctor form comes only after you complete account setup. You will need to fill out all required information, then you will have access to the documents section of your account.

"Missing Med+Proctor Physical Exam" What Does This Tag Mean?

This tag means that your school/ university requires you to receive a physical examination prior to registration. You must obtain a copy of your Med+Proctor Physical Exam Form to be filled and signed by your medical provider. This form can be accessed and downloaded by clicking the blue “Immunization Certificate” link on the “Downloads” page of your Med+Proctor account.

"Missing Men A after 16th birthday" What Does This Tag Mean?

The tag on your account stating “missing Men A after your 16th birthday,” means that your university requires a Meningococcal A vaccine administered on or after your 16th birthday. 
If this tag is placed on your account, this is based off of the documentation uploaded to your account that you received your most recent Men A vaccination prior to your 16th birthday. If you have documentation of an updated Men A vaccination (administered after your 16th birthday) please upload it. If you do not, you will need to receive a Meningococcal A booster.

"Missing Meningitis Acknowledgement Form" What Does This Tag Mean?

Your account is missing the Meningitis Acknowledgment Form.

"Missing Office Number" What Does This Tag Mean?

Your immunization form is missing the office phone number. Please fill in all misssing information regarding your healthcare provider on the immunization form.

"Missing Office Stamp" What Does This Tag Mean?

Your immunization form is missing the office stamp. Please take your form to your healthcare provider to get the required office stamp, then upload the updated forms back into your Med+Proctor account.

"Missing Reference Ranges" What Does This Tag Mean?

Your titer document(s) is missing the reference ranges. Reference ranges are used to compare the numerical results of titers to a range of values which determine immunity and non-immunity. Please be sure all titer documentation  include the numerical results as well as the numerical reference ranges. If you are unsure what a reference range is, please contact the healthcare provider or lab where you receieved the titer and ask for documentation with reference ranges.

"Missing Required Doc" What Does This Tag Mean?

Some schools require that your have a healthcare provider fill out and submit the immunization forms found on your Med+Proctor account. If you are seeing this tag it means you did not submit the document required by your school. The required forms can be found on the “Documents” page of your account, it will be a blue downloadable PDF link.

"Missing Student ID" What Does This Tag Mean?

You are required to enter a student ID issued by your school. If you are seeing this tag then we did not receive your student ID. This can be entered on the “Term” page of your Med+Proctor account.

"Parent Signature" What Does This Tag Mean?

Your immunization form is missing a guardian/parent signature. Please have a parent or guardian sign your forms then re upload them to your Med+Proctor account.

"Titer Non-Compliant" What Does This Tag Mean?

"Wrong Form" What Does This Tag Mean?

The form you uploaded is not the correct form. Your university requires that you use the immunization form found in your account. This form is located in the “Documents” section to download and print out.

"Wrong Program" What Does This Tag Mean?

Your program of study is incorrect. Please go back into your account and edit your program of study. Below is a link to login to your account to correct this error.

"Health Center Evaluation Needed" What does this tag mean?

If your account is tagged with this your university has been notified to review your account/records for compliance. You will be notified if any needs to be on your end. These emails will go to your university email address or the email on file with your university.

"Medical Exemption" What does this tag mean?

Your medical exemption has been received and your school has been made aware of its submission. Nothing is needed from you regarding your medical exemption at this moment, but you will be notified if any action needs to be taken on your end.

"Meningococcal A Expired" What does this tag mean?

If your account is tagged with this, you will need to upload an updated dose of Meningococcal/MCV4/Meningitis. Please note the difference between Men A and Men B. Your updated dose must be from within the last 5 years.

"MMR medical exemption" What does this tag mean?

Your MMR medical exemption request has been sent to your institution so they are now aware of your exemption. You will receive an email shortly regarding its approval.

"Religious exemption" What does this tag mean?

Your religious exemption has been sent to your institution so they are now aware of your exemption. You will receive an email shortly regarding its approval.

"Missing TDAP Vaccine Within 10 Years" What Does This Tag Mean?

Most state and school requirements for Tetanus mandate that one dose of Tetanus must not be older than 10 years. If you have this tag on your account, your Tetanus shot is expired and you are required to upload an updated TDAP vaccine from within the last 10 years in order to become medically compliant. You can speak with your provider, a clinic (like CVS or Walgreens) or any practice that administers vaccines to receive a TDAP booster shot.

"Missing 2 Prior Tetanus Dates" What Does This Tag Mean?

If your account was flagged with this tag, your school is requiring you to upload two more doses of Tetanus. The state requires proof of 2 prior Tetanus shot dates in your past medical history.

Once you have proof of these vaccines, please upload it to your Med+Proctor account so that our team can process and review your account.

"Missing Physical Exam Form" What Does This Tag Mean?

If your account has been flagged for a “Missing Physical Exam Form,” it’s essential to promptly submit the required physical exam form to meet your school’s medical standards. Once you have the form, you can conveniently upload it by scanning it, taking a photo with your phone, or emailing it to yourself for upload from a computer.

"Future Dates Not Accepted" What Does This Tag Mean?

If your account was flagged with this tag, one of your uploaded vaccine dates is a future date, which we cannot accept. Please upload an updated vaccine date so our staff can review and process your account appropriately.

"Temporary Waiver Request" What Does This Tag Mean?

he tag “Temporary Waiver Request” means your waiver request has been sent to your school to review and process. Please keep an eye out for an email with further instructions.

What is the Varicella Vaccine?

The varicella vaccine protects against chickenpox (varicella), a common and very contagious childhood viral illness. It also protects against shingles. A complete Varicella vaccination series consists of two vaccinations.

Varicella vaccinations are most commonly listed on your immunization records as: Varicella Zoster, VZV, Varivax, Chicken Pox, MMRV, Proquad, Varilrix

UW – Missing Provider Signature

A document you submitted is missing a healthcare provider’s signature and/or other information (address, license number, or phone number), which is needed to verify your immunizations.

Official immunization records such as PDFs from MyChart, MyIR documents, or pharmacy immunization receipts are acceptable without a signature.

Please resubmit with a healthcare provider’s signature or submit other acceptable records.

UW – MyChart Screenshot No Name

Your full name is not on the screenshot you submitted.

To obtain official immunization documentation, please return to your MyChart account and find your immunizations. At the top of the page with your immunizations, click on the printer icon. It will generate a PDF document that will show your name, date of birth, and immunizations.

If you prefer to send only the immunizations requested, you can click ‘View Details’ on the required vaccinations and click on the printer icon there. Please save those documents and resubmit them to your account.

If you submitted a screenshot of your titer results, please find your titers on your MyChart account again. On your lab test, click on the printer icon, and it will generate a PDF document that will show your name, date of birth, and titer test results. Please save those documents and resubmit them to your account.

The documents you submitted do not include proof of Meningitis ACWY vaccination. The University of Washington requires that students receive a Meningitis ACWY vaccine at age 16 or older.

Although the Meningitis B vaccine is recommended for students, it does not fulfill your Meningitis ACWY immunity requirement. The University of Washington requires that students receive a Meningitis ACWY vaccine at age 16 or older.

You have submitted proof of only one valid MMR (Measles, Mumps, Rubella) vaccine, or one Measles and one Mumps vaccine.

The University of Washington requires proof of two valid MMR vaccines, two Measles and Mumps vaccines, or positive results of both Measles and Mumps titer tests. You must receive a second vaccine or titer test 28 days or more after your most recent vaccine. If your titer test is equivocal or negative, you will need to receive a second MMR vaccine.

You received a Measles and/or Mumps vaccine, or MMR (Measles, Mumps, Rubella) vaccine, before you were 1 year old, which will not fulfill your requirement. You are required to show proof of two valid Measles and Mumps vaccines, or positive results of both Measles and Mumps titer tests.

Please submit proof of another valid MMR vaccine, or positive Measles and Mumps titer test results.

Please resubmit with documentation of one additional Measles or MMR (Measles, Mumps, Rubella) vaccine, or a positive Measles titer test, taken at least 28 days after your most recent vaccine.

UW – Needs 1 Mumps
